Yep, that is it.  We had the mail_always flag set.  Turning that off solved
the problem.

That's very strange.  Since sudo execs the the program to run it
can no longer be running once the other program has taken over.
Are you, by any chance, configuring sudo to send mail for ever
command?  In that case it would fork to send the mail and perhaps
that is where something is getting confused.
