Sounds like a PAM problem of some sort.  If I understand you correctly
the failed login count only increases when sudo prompts you for a
password?  If so, this could be related bug #150 in sudo sudo
bugzilla db.

I'm guessing it is related in some way to ipam_acct_mgmt() and
pam_tally but I really don't know.  Unfortunately, the PAM docs
don't shed any light on the problem.
